Due to the overwhelming number of insurance plans available to our patients, it is impossible for our staff to know the covered benefits of each and every plan. We do our very best to help you understand your coverage, but ultimately it is our patient's responsibility to know and understand the policies and benefits of their plan, including referrals, authorizations, co-payments, deductibles, covered hospitals, labs, and X-ray facilities.
In order for us to bill your insurance, we must be in-network. We ask that you provide a copy of your current insurance card at the time of your visit. If you can't provide the necessary insurance information, you will have to pay at the time of your service. If you do not wish to pay, your appointment can be rescheduled to a time when you can provide your insurance card.
Please make sure that both your physician and our hospital are listed as a contracted provider and facility by your insurance company. It may be possible that only your physician or our hospital is contracted with your plan. If not listed, contact your plan's customer service department for assistance.
